Hospitalization for a suicide attempt or suicidal thoughts can feel isolating and you may not know what comes next. Studies have shown that people are at the highest risk for suicide attempts and death in the 30 days following hospital discharge or release from the emergency department after a suicide attempt.
The Hospital Follow-Up Program (HFUP) offers free support for people recently discharged from a hospitalization for a suicide attempt and is funded by the Zero Suicide Initiative.
HFUP addresses the following topics through caring contacts by phone:
- Assessing suicide risk
- Developing a safety plan
- Connecting people to mental health and supportive services
- Encouraging people to engage in self-care
- Offering positive and emotional support
- and much more
These services are free, short-term, voluntary, and only available for residents of all ages in Alameda County. Our HFUP supervisor provides care offering a non-judgmental, collaborative, and compassionate approach. Bilingual/bicultural Spanish-speaking care is available at this time.
CSS partners with John George Psychiatric Hospital and Alameda County Behavioral Health Care Services (BHCS) to provide post-discharge follow-up care. We welcome referrals from other psychiatric facilities within Alameda County.
